Treating complex eye injuries and diseases often needs multiple eye care doctors working together. Our Vision Care Services Coordination program helps to tackle this challenge. Here is how it works:
Care Coordination
- VCSCs work with case managers, nurses, social workers, and medical providers. Their goal is to meet the eye care needs of service members.
- They are based at our four Ocular Trauma CentersGoes to OTCs.
- They work with eligible veterans and beneficiaries. This fact sheetView VCSC fact sheet gives an overview of how our VCSCs support vision care.
- They provide educational information and support for those who need specialized eye care.
Support They Offer
- Coordination for vision care
- Transition assistance to VA for vision rehabilitation services
- Coordination and eligibility confirmation for civilian ocular trauma and burn treatment
- Monitoring inpatient/outpatient care for parents and premature babies at risk for retinopathy of prematurity (ROP)
- Assistance for eligible veterans with referrals from DOD to VA medical services
- Support for vision rehabilitation to include adaptive technology and training
