If your child receives a vaccine outside of the military treatment facility, remember to get a record of it and share the information with your primary care manager or military clinic. This will ensure your child’s vaccination information is entered in a military electronic tracking system.
Incomplete documentation can lead to your child receiving duplicate vaccines or having the appearance of not being up to date on vaccines when he or she starts school or day care.
Proper records of vaccines administered by a civilian provider or at a TRICARE network pharmacy should include:
- Date the vaccine was given
- Vaccine name or code
- Manufacturer
- Lot number
- Volume of the dose given
- Name of prescriber
