Verification of Training

Residency and Fellowship training verification is provided by the individual GME programs.  We kindly ask that you email the program directly and provide the following information/documents in your email:

• Full name of the physician
• Program Name
• Graduation Date
• Your agency-specific verification of training form
• A signed "Authorization to Release Information" form signed by the physician

Two separate request packages for training verification are needed to be submitted for physicians completing a non-categorical PGY-1 year and subsequent residency training at NMCSD.

Example:  An institution requesting training verification of a radiologist would need to submit a request for training verification to the individual's PGY-1 program to verify completion of the PGY-1 year of training in addition to the training verification request to the Radiology Residency program.

**NMCSD's Pathology Residency program officially closed in 2023. We have transferred all of the training records to the Internal Medicine Department and they will be able to provide training verifications for former NMCSD Pathology Residents.
