Verification of Training

For the fastest response, please send all documentation electronically to the GME Office at dha.cavazos.darnall-amc.list.crdamc-gme-owners@health.mil.

Please attach all documentation forms completed to the greatest extent possible, including full name, your date of birth (if documentation requests it), start and end dates of training, identification numbers required for the documentation (NPI number, specialty board identification number, etc.) and your authorization to release information form. If the documentation is incomplete, the GME Office will reach back to you with a request to complete all elements before returning to us.

Again, please include all documents required to verify your training and include a return address so we can ensure we send your training verification documents to the proper agencies.
