Primary Care Behavioral Health provides:
- A team-based approach to health care by integrating behavioral health professionals into primary care
- Evidence-based, whole-person assessment, treatment, and prevention of health conditions
Behavioral health consultants work with patients in primary care to improve health, functioning, and quality of life. The Psychological Health Center of Excellence makes available a variety of assessment tools and handouts for use by patients who are working with a behavioral health consultant in primary care. These assessment tools and handouts are not designed as stand-alone, self-help materials, nor are they intended for use in specialty behavioral health settings. The following tools are currently available for download.
